Short-term Care - Personalized Rehab and Recovery Care

Helping you return home, at the highest level and earliest timeframe possible, are the goals of our short-term rehabilitation and recovery program.

Our short-term care services are for people of all ages. Our patient care team oversees your recovery from the acute phase of illness, injury or surgery to coordination of home care services and regular follow-ups after you leave our center.

As you prepare to transition back to your home, one of our rehabilitation therapists will complete a mock setup of your home to prepare you for a safe and independent return.

If you are planning selective surgery, please call our Admissions Department for rehabilitation services as soon as your surgery is scheduled.

Middlebrook At Middlesex Health Care Center:
Our Secure Dementia Care Unit

People with dementia need more than special care. Middlebrook at Middlesex Health Care Center specializes in caring for people who have been diagnosed with Alzheimer’s disease and other related dementias. Our 30-bed unit offers security and safety, as well as programming and recreational activities. We address each resident’s cognitive, physical, and emotional needs, desire for independence and opportunities for enhanced quality of life.

Our team includes physicians with gerontology and behavioral specialties, nurses, specialized nursing assistants, occupational and physical therapists, social workers, dietitians, and a therapeutic recreation specialist.
